Preparing for discharge
Our Case Management Department assists our patients through a hospitalization, whether it is acute care or swing bed, to ensure that all available resources are coordinated toward a patient's recovery.
Our Case Manager collaborates with the entire professional health care team available at Brookings Health System.
These services can include:
- Referrals to therapies
- Utilization of community resources after a hospital stay such as Meals on Wheels or homemaking/bathing services or finding support groups
- Arranging of home equipment needed
- Easing transitions to post-hospital services such as nursing home, assisted-living, skilled home health, cardiac rehabilitation, or hospice.
Our caring staff is here to assist our patients and their families who may be experiencing a difficult, stressful, or even life-changing event.
